Dr. Peto explains how gum pocket depth is related to gum disease.

In order to determine the state of health of your gums, Dr. Peto will do a periodontal probe of your gums. Pockets occur when gum disease spreads below the gum line, causing the gum to detach from the root surface and form pockets in which bacteria can multiply and cause further detachment.

Pockets that are 3 mm to 4 mm indicate moderate gum disease. A pocket that is 5 mm or deeper indicates severe gum disease (severe periodontitis).

Our experienced hygienists effectively treat gum disease with deep cleanings.

Our first line of attack on gum disease is a deep cleaning — which involves scaling and root planing. Our hygienists use ultrasonic scalers to thoroughly remove the plaque and accumulated calculus on the tooth surfaces above and below the gum line. They also smooth or plane away any pits, abrasions or rough areas on the root surface. This makes it easier for the gums to reattach, and makes it harder for plaque to attach to the surface and multiply.

Our hygienists have specialized training in treating gum disease and have worked 15 to 20 years exclusively in periodontal offices. They know how to properly treat periodontal issues and can often take care of gum disease with just a very thorough deep cleaning.

Your home oral hygiene is vital for the success of any treatment. We’ll provide you extensive education on how to best keep your gums free of disease. You’ll learn which toothbrush and floss are the most effective, and we’ll provide you information about other hygiene tools to use and how to use them. We also advise you on diets that will help you combat gum disease.

Cutting Edge Treatment with LANAP® Laser Therapy

If your gum disease is too severe or fails to respond to deep cleaning, we use LANAP (Laser-Assisted New Attachment Procedure). We are the only periodontist practice in Beverly Hills that utilizes this state-of-the-art technology. Dr. Peto uses the LANAP procedure to clear away diseased tissue without cutting or stitching of the gums. There is little to no post surgical discomfort. Dr. Peto has found the minimally invasive laser to be very effective in treating most cases of severe gum disease.

The aim of any gum disease treatment is to eliminate the bacteria and reduce the gum pockets caused by the bacteria attacking below the gum line. A tiny laser fiber (about the thickness of three human hairs) is inserted between the tooth and the gum, and the infection and disease are cleared away without cutting the gums with a scalpel. The tooth roots are then thoroughly cleaned of tartar and plaque, and the laser is used at a different setting to seal the gum pockets around the teeth. The procedure also stimulates the body’s natural regenerative potential.

Periodontal Surgery

In a few cases, Dr. Peto will need to perform conventional periodontal surgery, generally known as “flap” surgery. This is mostly for upper back teeth (molars) that have severe bone loss and a 9 mm plus pocket.

During this procedure the gums are lifted back, the tartar removed and the irregular surfaces of the bone smoothed out to limit areas where bacteria can attach. The gum then reattaches to the tooth, eliminating any pockets where the bacteria can multiply.

Antibiotic Therapy

In severe cases, Dr. Peto will recommend a low dose of antibiotic to improve tissue health. He may also prescribe antibiotics after surgery to prevent infection.

Gum Grafting

Gum disease can sometimes cause the gums to recede and expose the tooth roots. This increases tooth sensitivity and exposes the tooth root to the erosive effects of bacterial plaque, food acids and aggressive brushing.

In order to keep your gums healthy, we set you up on a periodontal maintenance schedule. We will do regular probes of your pockets to check the healing process, do thorough cleanings to remove bacteria, and work with you to keep you on a good home care regimen.
